Target Audience

This activity is intended for physicians, nurses, social workers, and licensed professional counselors.

Statement of Need

This presentation will cover how easily a cognitive assessment can be performed during the Medicare Annual Wellness visit.  Visit the algorithm on the Alzheimer's Association website.

Educational Objectives

Upon completion of this activity learners should be able to:

  • Review the Medicare Annual Wellness visit algorithm for assessment of cognition;
  • Review cognitive assessment tools;
  • Apply the Medicare Annual Wellness Visit algorithm to primary care patients; and
  • Demonstrate the Alzheimer's Association app
